P0174 indicates that the engine control module (ECM) has detected a lean air-fuel mixture on Bank 2 (the side of the engine opposite cylinder #1). This means there is too much air or too little fuel entering the combustion chamber, which can cause performance issues and potential engine damage if left uncorrected.

1. Vacuum/intake air leak on Bank 2

Unmetered air entering the engine after the mass airflow (MAF) sensor is the most common cause. Leaks can occur at intake manifold gaskets, vacuum hoses, or the PCV system on Bank 2.

3. Fuel delivery problem on Bank 2

4. Exhaust leak before the oxygen sensor on Bank 2

An exhaust leak upstream of the Bank 2 oxygen sensor can allow outside air to enter, causing the sensor to read lean and triggering the code.

5. Faulty oxygen sensor (O2) on Bank 2

A degraded or contaminated O2 sensor may send an incorrect lean signal to the ECM, but this is less common than an actual air or fuel issue. The sensor should be tested rather than replaced first.

6. Engine control module (PCM/ECM) issue

PCM failure is extremely rare for P0174. Replacing the computer is almost never the correct fix; the underlying mechanical or sensor problem should be thoroughly diagnosed first.

  1. Inspect for vacuum leaks on Bank 2 using a smoke machine or propane enrichment method, paying close attention to intake gaskets and hoses.
  2. Clean and test the MAF sensor with a multimeter or scan tool data; compare airflow readings to specifications.
  3. Check fuel pressure with a gauge and verify it is within spec; test fuel injector pulse and resistance on Bank 2.
  4. Monitor live oxygen sensor data for Bank 2; look for slow response or stuck lean voltage (below 0.45 V).
  5. Inspect the exhaust manifold and pipes on Bank 2 for leaks that could affect O2 sensor readings.
  6. If all other checks pass, perform a fuel trim reset and test drive; only consider PCM replacement after exhausting all other possibilities.
