P0387 indicates the powertrain control module (PCM) has detected an abnormally low voltage signal from the Crankshaft Position (CKP) Sensor 'B' circuit. Many engines use two CKP sensors—one as the primary timing reference and a second for misfire detection, cylinder identification, or backup—and this code points specifically to the secondary sensor reading below the expected threshold.

What causes it — most likely first

1. Failed CKP Sensor B itself

The sensor's internal Hall-effect or magnetic element degrades, shorts internally, or loses sensitivity, producing a low or absent signal. This is the single most common cause of P0387.

2. Damaged wiring or connector to CKP Sensor B

Chafed, heat-soaked, or oil-contaminated wiring and corroded, loose, or backed-out pins in the sensor connector are frequent, especially on harnesses routed near the lower block, bellhousing, or oil pan.

3. Damaged or contaminated reluctor ring (tone wheel)

Rust, debris, a missing tooth, or excessive runout on the crankshaft tone wheel that the sensor reads can drop the signal amplitude below the PCM's threshold.

4. Excessive sensor air gap or loose mounting

If the sensor was installed loosely, its bracket is cracked, or the mounting bolt is over- or under-torqued, the gap between the sensor tip and the tone wheel can be too large, weakening the induced signal.

5. Shared ground or 5-volt reference fault

A corroded engine ground strap or a short-to-ground in the sensor's reference-voltage wiring can pull the CKP B signal low even when the sensor is good.

6. PCM internal input-driver fault (rare)

The powertrain control module itself has a failed input stage for the CKP B circuit. Genuine PCM failures are uncommon for this code and should be considered only after the sensor, harness, and tone wheel have all been ruled out.

How it's diagnosed

  1. Retrieve all stored DTCs and freeze-frame data with a scan tool to confirm P0387 is current and note any related codes such as P0385, P0335, P0016, or cylinder-misfire codes.
  2. Visually inspect the CKP Sensor B connector and the harness along the lower block, bellhousing, and oil pan rail for chafing, oil contamination, heat damage, corrosion, and pin push-out.
  3. With the key on, back-probe the connector to check reference voltage and ground; while cranking, view the sensor's output on a lab scope to confirm a clean square wave (Hall-effect) or sine wave (variable-reluctance) of the expected amplitude.
  4. Measure the sensor's resistance or current draw and compare to the manufacturer's specification; readings outside the specified range confirm a failed sensor.
  5. Remove the sensor and inspect the reluctor ring through the bore for missing teeth, rust, debris, or excessive runout.
  6. If the sensor, tone wheel, and visible wiring are all good, perform a full circuit integrity test (continuity and short-to-ground/short-to-voltage checks on all three wires) before considering a PCM concern.
