P0316 indicates that the powertrain control module detected one or more engine misfires during the first 1,000 crankshaft revolutions after a cold start, before the catalytic converter reaches operating temperature. This startup-only misfire monitor is designed to protect the catalyst from raw fuel and excessive heat damage during warm-up. The code may appear alone or with cylinder-specific misfire codes such as P0301–P0308.

What causes it — most likely first

1. Worn or fouled spark plugs and related ignition components

Spark plugs, ignition coils, and plug wires degrade over time and are the most common source of cold-start misfires because a richer mixture and lower cylinder temperatures make ignition harder during warm-up.

2. Vacuum or intake-air leaks

Unmetered air entering downstream of the mass airflow sensor creates a lean air-fuel mixture that is more likely to misfire at low engine speeds and before the oxygen sensors are fully active.

3. Fuel-delivery problems

A weak fuel pump, clogged filter, leaking injectors, or dirty injector nozzles can cause lean or rich misfires that are most noticeable when the engine is cold and operating in open-loop fuel control.

4. Engine mechanical or low-compression condition

Worn rings, leaking valves, a jumped timing chain or belt, or a blown head gasket can reduce cylinder sealing and produce consistent misfires that show up immediately on startup.

5. Faulty engine sensors or wiring

Inaccurate coolant-temperature, crankshaft-position, camshaft-position, or mass-airflow sensor readings can disrupt fuel and spark timing just long enough to trigger startup misfire detection.

6. Powertrain control module fault

A PCM issue is genuinely rare for this code; it should be considered only after all ignition, fuel, mechanical, and sensor causes have been ruled out and the wiring harness is verified.

How it's diagnosed

  1. Retrieve all stored and pending codes, record freeze-frame data, and note whether cylinder-specific misfire codes accompany P0316.
  2. Perform a visual inspection of spark plugs, ignition coils, plug wires, vacuum hoses, intake gaskets, and the air-induction system for damage, carbon tracking, or leaks.
  3. Test ignition output and compare coil performance across cylinders; inspect plugs for oil, coolant, carbon, or abnormal wear patterns.
  4. Check fuel pressure at key-on, idle, and under load, and evaluate injector spray patterns or perform an injector balance test as applicable.
  5. Run a relative or absolute compression test and verify valve timing if mechanical failure is suspected.
  6. Inspect sensor data with a scan tool during cold-start, and check sensor circuits for correct voltage, ground, and signal integrity before considering module replacement.

Once warm-up is complete, the same underlying fault may store as a generic P0300 or cylinder-specific P030x code instead.
