P0166 indicates the powertrain control module (PCM) has detected little or no voltage activity from the oxygen sensor located after the catalytic converter on Bank 2, position Sensor 3. The sensor signal is staying essentially flat instead of varying with exhaust oxygen content. This usually points to a problem with the sensor, its wiring, or the exhaust stream reaching the sensor, rather than the PCM itself.

1. Failed Bank 2 Sensor 3 oxygen sensor

The internal sensing element or heater circuit can degrade with age and heat, causing the sensor to stop producing a meaningful voltage signal.

2. Wiring or connector fault in the sensor circuit

Open, shorted, or corroded wires and poor terminal connections can interrupt the signal or reference voltage path, making the PCM see no activity.

3. Exhaust leak near the sensor

Air drawn in through a leak ahead of or at the sensor boss can dilute exhaust gases and prevent the normal voltage switching the PCM expects.

4. Contaminated or aging sensor

Coolant, oil, silicone, or fuel additives can coat the sensor element over time, slowing or stopping its response even before complete electrical failure.

5. PCM signal-processing fault

This is uncommon; the PCM rarely fails to read only one oxygen sensor, and should be considered only after the sensor, wiring, and exhaust have been ruled out.

How it's diagnosed

  1. Confirm P0166 and any related codes with a scan tool, then inspect the sensor connector, harness, and exhaust for visible damage, corrosion, or leaks.
  2. View live data for Bank 2 Sensor 3 voltage; a healthy downstream sensor should show some variation, while a stuck flat signal suggests sensor or circuit failure.
  3. Compare Bank 2 Sensor 3 behavior with Bank 1 Sensor 3 under similar driving conditions to determine whether the issue is isolated to one bank.
  4. Check the sensor heater power and ground circuits, since a cold sensor may not produce a usable signal; also test signal-wire continuity and resistance back to the PCM.
  5. Perform a propane enrichment or snap-throttle test to see if the sensor voltage responds at all; no response strongly indicates a failed sensor or wiring break.
  6. If the sensor, wiring, and exhaust are all verified good, evaluate the PCM only as a last resort before replacement.
