Cylinder 11 is only present on engines with 11 or more cylinders (most commonly V12 configurations), so this code is relatively uncommon.

What causes it — most likely first

1. Failed cylinder 11 fuel injector

An internally shorted or open-coil injector is the single most common cause. Injectors live in a hot, vibration-heavy environment and their solenoid windings degrade over time.

2. Damaged or corroded injector wiring/connector

Chafed insulation, heat damage, or spread/corroded pins in the injector connector create high resistance or intermittent open circuits that the PCM interprets as low voltage.

3. Open or shorted injector harness

A break in the conductor or a short to ground between the PCM and the cylinder 11 injector will drop circuit voltage below the expected range.

4. Poor ground path for the injector

Corroded engine grounds or a weak chassis ground can pull the low-side driver voltage low enough to trigger the code.

5. PCM injector driver issue

Occasionally an output driver inside the PCM that switches cylinder 11 fails or becomes weak. This is plausible but uncommon and should only be considered after wiring and injector are ruled out.

6. Failed PCM/ECM

Total control-module failure is rare for this code. Most vehicles diagnosed with P0291 and a 'replaced PCM' outcome actually had a wiring or injector fault that was missed.

How it's diagnosed

  1. Verify the code with a scan tool and review freeze-frame data (RPM, load, ECT) to confirm the fault is consistent and not a one-time event.
  2. Perform a visual and tactile inspection of the cylinder 11 injector connector and harness, looking for chafing, heat damage, oil saturation, bent pins, or loose fit.
  3. Measure the injector coil resistance with a multimeter and compare to the manufacturer specification; an out-of-spec reading points to a failed injector.
  4. With the ignition on (engine off), backprobe the injector connector to verify battery voltage is present on the feed side and a clean ground on the low side.
  5. Use a noid light or oscilloscope to confirm the PCM is actually pulsing the cylinder 11 injector; if no pulse is seen, the fault is upstream (wiring or driver).
  6. Swap the cylinder 11 injector with a known-good adjacent cylinder, clear codes, and re-test; if the code moves to the new cylinder, the injector is the cause.
