P0257 indicates the powertrain control module (PCM) has detected the fuel metering control signal on the injection pump's 'B' circuit is outside its expected range or behaving erratically. The code applies primarily to diesel engines with electronic distributor/inline injection pumps that use a fuel metering actuator (solenoid or valve) to regulate injection quantity and timing.

What causes it — most likely first

1. Faulty fuel metering actuator or solenoid on the injection pump

The metering solenoid itself is the most common failure. Internal coil winding breakdown, sticking pintle, or worn internal components cause the duty cycle / current feedback to drift outside the calibrated range.

2. Wiring, connector, or harness fault in the metering control circuit

Chafed insulation, broken wires, corroded pins, water intrusion at the pump-side connector, or poor chassis grounds frequently produce this code before the pump itself is at fault.

3. Low or contaminated fuel supply to the injection pump

A weak lift/transfer pump, restricted fuel filter, water-in-fuel, or air ingress prevents the injection pump from developing required rail pressure, causing the metering actuator to overwork and flag a range/performance fault.

4. Internal wear of the injection pump (cam ring, rotor, or plungers)

Mechanical wear inside the pump changes fuel delivery characteristics so the actuator can no longer keep the actual quantity within the PCM's expected window, especially at idle or high load.

5. Sensor feedback issue (cam position, rail pressure, or IAT/ECT skewing demand)

If the PCM is acting on a false demand signal, it commands a metering position the pump cannot sustain, and a P0257 can be set even with a healthy pump.

6. PCM/ECM driver fault or software calibration issue

A failed output driver, internal solenoid control short, or an outdated calibration can set this code, but this is the least common cause and is frequently misdiagnosed. Always rule out the pump, wiring, and fuel system first.

How it's diagnosed

  1. Retrieve all stored DTCs and freeze-frame data; record the engine coolant temperature, RPM, and fuel pressure at the time of the fault.
  2. Verify fuel supply pressure and volume with a gauge at the pump inlet and the transfer/lift pump output, and inspect the filter and fuel for water or contamination.
  3. Visually inspect the injection pump harness and connector for chafing, heat damage, oil ingress, and pin condition; check grounds.
  4. Measure the fuel metering actuator solenoid resistance and isolation against the manufacturer specification, and command the actuator on with a scan tool while monitoring actual current/duty cycle versus commanded.
  5. Use a scan tool to graph rail pressure, desired metering position, and actual metering position together; persistent deviation confirms an actuator or internal pump issue.
  6. Check for applicable technical service bulletins for the specific engine before condemning the injection pump or PCM.

Do I need to replace the PCM for P0257?

Almost never. Can I continue driving with P0257?

It is not advisable. Many vehicles will enter a power-reduction mode, and continued operation with incorrect fuel metering can cause elevated exhaust temperatures, soot loading, and in some cases internal pump or engine damage. Diagnose and repair before extended driving.

Is P0257 a gasoline-engine code?

It is overwhelmingly a diesel code tied to electronic rotary or inline injection pumps. On port-fuel-injection gasoline engines the equivalent fuel metering faults use different DTCs, so seeing P0257 strongly suggests a diesel powertrain.
